1. Minimum of 20 employees as principal payors.
2. The Company must be registered at the Philippine Securities and Exchange Commission.
3. Any person who is 18 yrs. old up to 64 years old and regularly employed by the company.
4. Legal dependents of the Company Employee declared by the said employee as dependents (parents up to 64
years old, children from 3 months up to 21 yrs. old, unmarried and unemployed) eligible under PhilHealth and
accepted as Members under the Corporate Health Care Program.
