Family Program
Searching for health care program that would be suitable for your family? Fortune Care's Family Healthcare program is ideal for you! This program is specifically created to gratify the healthcare needs of the family. It features Out-patient Care, In- Patient Services, Preventive Care, Emergency Room Care and World wide Emergency Care. This also includes supplemental benefits such as (1) Daily Hospitalization Income Benefit, (2) Personal Accident and Dismemberment Benefits worth P10,000 Face Amount, and (3) Financial Assistance of P10,000 in case of natural death as part of the program.
*Dental Care Benefits are optional.
Eligibility Requirements
Any gainfully working person from 18 years to 64 years of age as of last birthday may be enrolled as Principal member. The program shall have one or several qualified dependents. However, enrollment shall follow the rule of hierarchy.
Enrolled dependent spouse and Parents must be below 64 years old as of last birthday.
Enrolled dependent children must be 3 months up to 21 years old as of last birthday.
For married payors:
He/She should enroll first his/her spouse and then eldest child, then second eldest and so on.
For single, payors:
He/She should enroll first his/her mother, then father.
Single Parents can only enroll his/her children, eldest child, then second eldest and so on.
The principal member may or may not be a member of PhilHealth. However, the corresponding membership rates and those of their dependents shall be based on membership or non-membership in the PhilHealth Program.
NOTE: The application of membership is subject to Medical Underwriting Evaluation and Approval.
