Group Program
Group Program is coverage of formal association of ten or more persons, with or without juridical personality, enrolling together. Members may be employees of corporation or business entity; members of association, cooperative or club; or other such groupings of like-minded purpose with one business address. They may enroll their dependents as additional but not as principal members.

Eligibility Requirements

Phil-Health Integrated Programs
       1. Principal Members- any gainfully employed Philippine resident; up to 64 yrs. old.
       2. Dependents
             a. Single Principal Payor
                    - Parents up to 64 years old
              b. Married Members
                    - Legal spouse up to 64 yrs. old
                    - Legal children from 3 months up to 21 yrs. old as of last birthday, unmarried
                      and not gainfully employed

Non-Phil-Health Integrated Programs
       1. Principal Payor- any gainfully employed Philippine resident, up to 64 years old, not covered by Phil-Health.
       2. Dependents
              a. Single Principal Payor
                     - Parents up to 64 years old 
                     - Brothers & Sisters from 3 months up to 21 years old; unmarried and not gainfully employed
              b. Married principal payor
                     - Legal spouse up to 64 years old
                     - Children from 3 months up to 21 years old as of last birthday

NOTE: The application of membership is subject to Medical Underwriting Evaluation and Approval.
