Senior Citizen Program
SENIOR CITIZEN PROGRAM is a special program for all Fortune CARE loyal Clients (ages 65 to 79 years old).
A program designed and offered to all existing clients with at least ten (10) years continuous membership with Fortune CARE. Said program provides wide range of health care services, which are categorized as Preventive Healthcare, Out-Patient Care, In-Patient Care, Emergency Room Care and Worldwide Emergency Care. Supplemental benefit include Third Party Liability of P 20,000.
Eligibility Requirements:
1. Fortune CARE member with ten(10)-year continuous membership with at least 65 years of age but not more than 79, may be enrolled as an individual member.
2. The legal spouse of the principal member.
3. The principal member and/ or his/ her spouse who is a member of PhilHealth program are both eligible.
NOTE: Membership application shall be subject to Medical Underwriting/Evaluation and Approval.
