Fortune CARE Long Weekend Reminder

Our Dear Valued Member: 

Thank you for choosing us to be your health care partner over the years. In our desire to ensure seamless availment of your purchased healthcare benefits with us during the upcoming long weekend, we would like to remind you about the following: 

1. All Fortune Care clinics will be closed on August 20 and 21, 2012 in celebration of End of Ramadan (Eid’l Fitr) and Ninoy Aquino Day, respectively. In this regard, for your emergency out-patient care, you may proceed to the nearest accredited hospital depending on your purchased plan. Regular operations will resume on August 22, 2012.

2.   Fortune Care Sales Office and Provincial Branches will be closed on August 20 and 21, 2012 in celebration of End of Ramadan (Eid’l Fitr) and Ninoy Aquino Day, respectively. In this regard, you may proceed to any of our accredited payment facilities (Bayad Centers, Fortune CARE online payment and Bancnet Automated Teller Machine) for updating of your membership fee and settlement of non-coverable charges to ensure continuous health care benefits. Regular operations will resume on August 22, 2012.

3.  In cases of emergency confinement during the long weekend, please ensure that all Philhealth requirements are duly filed upon discharge. Otherwise, you will be required to pay the Philhealth portion as your program with us is Philhealth integrated.

A. For Philhealth principal payor, please present Proof of payment for the past 6 months, Philhealth Form Nos. 1 & 2, Membership Data Record and/or Birth certificate.

B. For Philhealth dependents, please present Proof of payment for the past 6 months, Membership Data Record (MDR), Birth certificate for children or Marriage Certificate for legal spouse. In the absence of the MDR, they are required to present a photocopy of the Birth Certificate/ Marriage contract if applicable. You may go to the nearest Philhealth office for details of their requirement.

4.  For elective confinements or scheduled surgery, we encourage you to have it done before or after long weekend to prevent inconvenience at the time of availment. However, please ensure that all Philhealth requirements are duly filed  upon discharge. Otherwise, you will be required to pay the Philhealth portion as your program with us is Philhealth integrated.

5.  For any availment concerns, you may call our 24-hour membership assistance hotline: 1-800-10-633-8888 (provincial) or 706-4849 (Metro Manila). For members with Corporate program, you may coordinate with your assigned Customer Service Coordinators for assistance.

6.  For members under our corporate health programs, you may coordinate with your assigned Customer Service Coordinator for assistance.
